Does this work on scanned documents?

No. It reads text that is already stored in the PDF. A scanned or photographed page is just an image, so it comes out empty — that needs an OCR tool, which this is not.

Will the formatting be preserved?

This tool extracts raw plain text, so complex layout formatting like columns and tables may not be perfectly maintained.

Can I tell which page each part came from?

Yes. The .txt file includes a marker line before each page’s text, so you can trace a quote back to the right page.
